FAQ / services

We ask our patients to:

  • Be punctual for their appointments
  • Endeavour to follow advice given
  • Be polite to staff
  • Give due notice for cancellations
  • Bring proof of entitlement when claiming help with the cost of NHS treatment

Please note that we have the right to refuse treatment for patients who are rude, abusive or repeatedly miss appointments.

PaymentsPayments can be made by cash, cheque and debit or credit card. A deposit is required at the start of the treatment and fees should be paid in full on completion of treatment.

Patient information:
What information do we hold about you?

  • Your name, address and date of birth
  • Why you need care and treatment
  • Records of when you were seen and the treatment provided

Why do we have information about you?

  • To give you good care and treatment
  • To check our staff are working well
  • To plan new services

Do I have to give you my information?

  • We have to have at least basic details to treat you
  • If you have concerns about the information we require and why, please tell us and we can answer your concerns.

Who can see my information?

  • Those who provide you with care need to see information on your care and treatment. Administration staff need to know some of your details to manage the organisation.

Do we share your information with other people?

  • Sometimes we need to talk to other organisations to make sure you receive all the care you need. We will always talk to you about this before we talk to them. In exceptional circumstances we may have to disclose some information without your consent. This will only take place if there is a legal justification.

Your rights and responsibilities:

You are entitled to:

  • A thorough examination of your mouth, teeth and gums.
  • A full explanation of your treatment options and a written treatment plan.
  • Information about NHS charges.
  • Advice on how to keep your teeth and gums healthy.
  • A care and treatment summary if you decide to transfer to another dentist.
  • Make a complain if you are not happy with your treatment.
  • Request to see information about yourself.

Health and safety
Safety and cross infection control for patients and staff is paramount in our minds. We treat patients with the care and attention we would give our families.
We comply with all cross infection standards and are continually reviewing our practice to ensure that we meet the highest standards of practice. All instruments are cleaned and sterilised and where possible disposable equipment is used. All cups, needles, tissues, suction tips and gloves are disposed of after every patient.

Access
There is disabled access to the practice, with no steps leading to the entrance. The surgery and cloakroom are situated on the ground floor.

NHS Contract
The NHS contract is held by Bristol Primary Care Trust and performed by Dr Jan Skvarka
and Dr Jan Strapec.
